En este proyecto se aplicar\u00e1 el an\u00e1lisis microecon\u00f3mico y microeconom\u00e9trico para analizar emp\u00edricamente cu\u00e1les han sido los efectos de la reciente crisis econ\u00f3mica desde el punto de vista de la equidad y la eficiencia del Sistema Nacional de Salud en Espa\u00f1a (SNSE). Con la evidencia emp\u00edrica que obtengamos, el reto es el de guiar a las autoridades p\u00fablicas en el dise\u00f1o de intervenciones p\u00fablicas eficaces que ayuden a optimizar el uso de recursos en el SNSE en esta fase de recuperaci\u00f3n (a\u00fan con ajustes) que ya est\u00e1 afrontando el Estado Espa\u00f1ol. Este proyecto de investigaci\u00f3n viene dar continuidad a dos proyectos anteriores a los que han pertenecido buena parte de los miembros del equipo de investigaci\u00f3n del proyecto: ECO2012-36150 y SEJ2004-08073\/ECON. La investigaci\u00f3n se materializar\u00e1 a trav\u00e9s de cuatro l\u00edneas de investigaci\u00f3n: En primer lugar (l\u00ednea de investigaci\u00f3n 1), con base en diferentes oleadas de las Encuestas Nacionales de Salud ENS (MSSSI, 2006- 2016), se analizar\u00e1n los efectos de la crisis econ\u00f3mica sobre la equidad horizontal en la provisi\u00f3n sanitaria dentro del SNSE, tanto por niveles socioecon\u00f3micos (aproximado por nivel de renta, nivel de estudios y ocupaci\u00f3n) como por nacionalidad, distinguiendo el an\u00e1lisis a dos niveles: a) efecto de la crisis econ\u00f3mica en la equidad en la provisi\u00f3n sanitaria en el SNSE de los adultos; b) efecto de la crisis econ\u00f3mica en la salud y en la equidad en la provisi\u00f3n sanitaria en el SNS de dos grupos vulnerables: ni\u00f1os y ancianos. En segundo lugar (l\u00ednea de investigaci\u00f3n 2), se cuantificar\u00e1 y explicar\u00e1 el impacto que la crisis econ\u00f3mica ha tenido sobre la obesidad y su gradiente social en Espa\u00f1a y en la Comunidad Canaria. El estudio sobre obesidad que se plantea en esta l\u00ednea de investigaci\u00f3n tambi\u00e9n se llevar\u00e1 a cabo con datos de las ENS, aunque ser\u00e1n completados, para el caso de Canarias con los datos extra\u00eddos del cuestionario de la cohorte Cardiovascular, Diabetes y C\u00e1ncer (CDC) de Canarias (6729 adultos). En tercer lugar (l\u00ednea de investigaci\u00f3n 3), se analizar\u00e1, c\u00f3mo se ha visto afectada, tras el cambio en el copago farmac\u00e9utico (RDL 16\/2012) la distribuci\u00f3n de la financiaci\u00f3n privada de los medicamentos por grupos de usuarios del SNSE (activos y pensionistas). Adem\u00e1s, se analizar\u00e1 si tras dicho cambio, la demanda de medicamentos se ha resentido indistintamente en todos los grupos de medicamentos o, por el contrario, \u00e9sta se conseguido mantener en aquellos indicados para el tratamiento de enfermedades cr\u00f3nicas y\/o graves. Se dispone de una muestra aleatoria representativa de la poblaci\u00f3n con tarjeta sanitaria individual de la Comunidad Aut\u00f3noma de Canarias (64.000 personas), estratificada por siete \u00e1reas de salud y condici\u00f3n de activo o pensionista, que permite hacer un an\u00e1lisis before and after del cambio producido en 2012. En cuarto lugar (l\u00ednea de investigaci\u00f3n 4), se llevar\u00e1 a cabo un an\u00e1lisis del efecto que la crisis econ\u00f3mica ha tenido sobre la eficiencia y productividad de los hospitales del SNSE. Para ello, se plantear\u00e1 la estimaci\u00f3n de una frontera estoc\u00e1stica de costes multiproducto, bajo una forma funcional flexible, que posibilitar\u00e1 una caracterizaci\u00f3n completa de la tecnolog\u00eda de producci\u00f3n de la actividad hospitalaria. Para ello se utilizar\u00e1 la informaci\u00f3n sobre recursos, actividad, costes e inversi\u00f3n (tanto de las unidades hospitalarias espa\u00f1olas como de sus centros de especialidades asociados) provenientes la Estad\u00edsticas ESCRI y CSDAE (MSSSI 2002-13).<\/p>\n

In this project, a micro-economic and micro-econometric analysis will be applied to empirically analyse the effects of the recent economic crisis from the viewpoints of equity and efficiency of the Spanish National Health Service (SNHS). With the empirical evidence obtained, the challenge is to guide health authorities to design effective public interventions that will help optimize the use of resources in the SNHS during this stage of recovery (still under fiscal adjustments) facing the Spanish state. This research project continues and completes the work of the two previous projects to which the majority of the projects research team belonged: Project ECO2012-36150 and Project SEJ2004-08073\/ECON. The objectives of this research projects follow four main lines of research: First (research line 1), based on the different waves of the Spanish National Health Surveys ENS (MSSSI, 2006-16), the effects of the economic crisis on horizontal equity in health care provision in the SSNS are analysed, both by socioeconomic levels (approached by income, education and occupation) and by nationality. This analysis will be differentiated at two levels: 1. Effect of economic crisis on equity in health provision for adults; 2. Effect of the economic crisis on health and equity in health care provision in the NHS on two vulnerable groups: children and the elderly. Second, (research line 2), the impact that the economic crisis has had on obesity and its social gradient will be explained and quantified for Spain and the Canary Islands. The study on obesity will be also conducted with data from ENS although for the case of the Canary Islands, data will be supplemented with data taken from the Cardiovascular Cohort, Diabetes and Cancer (CDC) questionnaire (6729 adults). Third (research line 3), an analysis will be made of how distribution of private funding of medicines by SNHS user groups (active users and pensioners) has been affected, following the change in the pharmaceutical copayment (RDL 16\/2012). In addition, following the change, there will be an analysis of whether the demand for drugs has suffered equally in all groups of drugs or, on the contrary, the demand for those with treatment of chronic and\/or serious illness has been maintained. A representative random sample of the population with individual health cards of the Canary Islands (64,000 people) will be used, stratified into seven areas of health and condition of active users or pensioners, which allow us to undertake a before and after analysis relative to the 2012 copayment change. Fourth (research line 4), an analysis will be conducted of the effect that the economic crisis has had on the efficiency and productivity of hospitals in the Spanish National Health Service. To do this, the estimation of a stochastic multiproduct cost frontier under a flexible functional form will be made. This will allow a complete characterization of the production technology of hospital activity. Resource information, activity and investment costs (both Spanish hospital units and their associated specialist centers) will be used from the following statistics: ESCRI y CSDAE (MSSSI 2002-13).<\/p>\n
