• Sworn statement regarding disqualification (Spain)
• Sworn statement regarding disqualification (for foreigners)
• Sworn statement regarding incompatibility (except for associates or welfare associates).
• Model of sworn statement (associates and welfare associates).
• Request for compatibility with the performance of other public or private activities (.pdf) (electronic headquarters)
• – If it is a private activity, you must provide certification of the schedule from the private company (if it is self-employed, a sworn statement specifying the schedule)
• – If it is a public activity, you must request compatibility from the other organization and provide a photocopy of the application so that we can send the relevant documentation from here.
